An intelligence failure led to Israel’s deadly attack on civilians in the southern Lebanese village of Kana, an Israeli army inquiry found. The army said it fired missiles because intelligence had indicated that Hezbollah fighters were sheltered in the building, not civilians. Meanwhile, a Lebanese hospital revised the death toll from the attack to 28.
